Coaching Applications and Bench Staff Forms

If you want to Coach with AMLA, the Coaching Application is located here for downloading. As well, if you haven't had a new Police Check done, that is required before you will be allowed to work with a team. This letter goes to the OPP to get your Police Check done as part of AMLA. Since we have not been in operation since 2019 all Bench Personnel and Executive and any volunteers will need to do a new CRC unless you have had one completed within the last 12 months of the beginning of our lacrosse season. Declarations will not be accepted for the 2022 season because of the break in play.
